Central City Integrated Health is committed to enabling every patient in our community to thrive by building a collaborative multidisciplinary healthcare team. The Outreach Facilitator accomplishes treatment needs by developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches and monitoring and addressing social determinants of health and housing. Also responsible for establishing trusting relationships with patients and their families while providing necessary linkages to community resources with the goal of reducing healthcare disparities. The Outreach Facilitator must work within the parameters outlined but the dictates of the funding source and must demonstrate the following personal and physical qualifications, which are essential functions of the position.

Required Education/Experience

• Completion of higher education coursework in Social Work or another human services field, or possesses a combination of education and experience, which provide the necessary abilities, qualities and skills listed

• Experience working with patients exhibiting severe and persistent mental illness

• Must be comfortable with community outreach.

Must maintain confidentiality relating to treatment in accordance with HIPAA guidelines.
