Overview:
The Clinical Pharmacist Informaticist position will work directly with the health care team and patients to provide safe and effective pharmacy services in addition to the dispensing of medications. The pharmacist will provide ambulatory pharmaceutical care services using approved guidelines in collaboration with other health professional, including managing patient drug therapy and providing patient education. In addition, this position would work closely with the Medical Director of Clinical Informatics and the Clinical Informatics team to optimize the Electronic Health Record for our ambulatory pharmacists, whether they are working within our clinical or retail environments. Goals would be to improve, streamline and assist with various processes such as documentation/ordering/refill/prior authorization workflows, maintenance tasks as they pertain to Surescripts, NCPDP, medication pricing, formularies, improving medication adherence, and be an integral team member in helping Pharmacy and Clinical Informatics support other clinical departments with meeting organizational objectives and goals.
Responsibilities:
  • Conducts patient medication history inquiries upon referral from physicians and other medical
  • Given a diagnosis, synthesize a treatment plan, which may include drug therapy and lifestyle modifications, and transition within the continuum of care
  • Identifies, resolves, and prevents potential and actual medication-related problems including: untreated indications, improper drug selection, sub-therapeutic dosage, overdose, failure to receive medication, adverse drug reactions, drug interactions, and medication without
  • Under protocol, initiates, adjusts, monitors, and recommends changes in drug therapy based on information obtained through patient interviews, objective monitoring parameters, review of drug profiles and the patient medical
  • Coordinates and interprets laboratory monitoring of drug therapy, including serum drug levels, electrolytes, and other approved drug-related laboratory
  • Provides patient monitoring for the purpose of evaluating and optimizing patient outcomes, drug utilization and patient compliance, detecting and/or minimizing ADR's, eliminating unnecessary drug use and
  • Provides patient education related to the individual's disease state, including predictable actions, side effects, reasons for proper compliance, and any special
  • Assure ongoing performance improvement of the pharmaceutical care services
  • Provide functional direction to care management team, analyze and resolve problems in order to assure high quality and efficient workflow to improve health

Qualifications:
  • Graduate of an ACPE accredited college of pharmacy with a Doctor of Pharmacy degree and a California pharmacist license required or be certified by the Foreign Pharmacy Graduate Equivalency Committee (FPGEC) issued by the National Association of Boards of Pharmacy
  • Master’s Degree in Clinical/Health Informatics preferred.
  • Minimum 5 years of experience as a practicing Pharmacist required.
  • Healthcare Information and Management Systems Society (HIMSS) certification, or other related certification preferred, in the absence of a Master’s Degree in Clinical/Health Informatics.
  • AHIC certification or eligibility to sit for the AHIC certification is preferred.
  • Board certification (BCACP) in ambulatory care or completion of a Pharmacy practice residency
  • Bilingual: English/Spanish/Mandarin/Cantonese preferred depending on location.
  • Previous experience with computer based systems required; Electronic Health Record (EHR) experience required. Prior EHR implementation experience preferred.
  • Certification from Epic preferred.
  • Bilingual: English/Spanish strongly
