Job Summary:
The pharmacy resident shall review all medication orders and assess the completeness and appropriateness of the medication profile prior to dispensing and administration. Except in emergency situations, all medication orders must be reviewed by a pharmacist prior to dispensing. All medication profiles must include the patient's height, weight and allergies, prescriber's ID number.
The resident always acts in the best interest of the patient.
Able to prepare all required drug formulations, dosage forms, strengths as required.
The pharmacy resident is expected to apply pharmacokinetic principles and recommend new regimens when patient variables indicate the need for change. Identify serum drug levels requiring dosage alteration and able to recommend appropriate changes as required. Demonstrate knowledge and ability to apply pharmacokinetic principles in complex patient care situations. Apriori dosing is appropriate; Interprets levels appropriately. Modifies doses appropriately. Writes progress notes per protocol
Provide oral and/or written consultations that define drug therapy regimens that are appropriate with respect to drug selection, dose, administration schedule, route of administration. Identifies appropriate monitoring needs of the patient and interprets relevant date to formulate a plan to assess the patient's response to therapy. Communicates directly with the primary physician(s) and provides consultative summaries and progress notes in the patient record.

Qualifications

REQUIRED:
The resident is a graduate of an ACPE-accredited Doctor of Pharmacy degree program.
The applicant shall be eligible for licensure in California.
The candidate agrees to adhere to the rules of resident matching program process.
The candidate is enrolled in the ASHP matching program.

Pay Range: $20.03 - $30.04

Individual wages are determined based upon a number of factors including, but not limited to, an individual’s qualifications and experience
