Summit Healthcare Services is growing! We have a one-of-a-kind unique continuum of care coupling Living Well (personal training), Outpatient Therapy, Skilled Home Health, Palliative and Hospice.

We are currently looking for per-diem Physical Therapists for our Home Health agency, to provide rehabilitative care for home health patients in their private apartments in long-term care facilities as well as in their own private residences in the community at large. This position will take place in the west valley including Phoenix, Peoria, Sun City, etc.

This is the best Home Health job out there: 1 patient per hour relaxed atmosphere with no productivity requirement seeing 6 to 7 patients per day. Flexible schedule you make yourself which results in a great work/life balance with no weekends, no overtime and paid holidays even! Since it is per diem in nature, you can choose to pick up as many or as little visits as you'd like. $90-$100 per visit plus mileage reimbursement.

Job Requirements:

• Professionally Licensed in AZ
• Home Health experience of at least a year
• CPR Certification
• Fingerprint Clearance Card
• Valid driver’s license and proof of auto insurance

Pay: $90.00 - $100.00 per hour
