ESSENTIAL FUNCTIONS:
- Assess the well-being of clients and mitigate any identified risk with direct supervisor and other appropriate team members.
- Assess referral information for clients and determine if an appropriate match with a foster/kinship/adoptive family can be made.
- Communicate to the referral source availability of bed space for clients referred.
- Maintain a positive working relationship with all referral sources and provide open bed lists regularly for potential clients.
- Coordinate and/or participate in pre-placement activity for referred clients.
- Complete or coordinate assessments to ensure client is authorized for the appropriate level of care.
- Coordinate and/or place referred clients
- Complete intake paperwork in our electronic records system. Update system with all referrals within one business day of receiving referral.
- Assure all documentation meets state, contract, accreditation, and The Bair Foundation requirements.
- Assure the program is placing a minimum of 8 children/month and/or in accordance with census goals.
- Carry a limited caseload of clients and families which is subject to increase depending on intake productivity or lack there of.
- Responsible for all tasks associated with TC position which include:
- Ensuring the safety of all children served.
- Ensure that abuse and neglect concerns are appropriately reported and immediate actions are taken to ensure client safety.
- Assess the well-being of clients and mitigating any identified risk with direct supervisor and other appropriate team members.
- Monitor progress of children in the foster/kinship home setting.
- Monitor compliance of the foster/kinship home and safety of the child(ren).
- Provide direct supervision to assigned foster/kinship families, including but not limited to coaching, follow-up and accountability.
- Provide on-going training to foster/kinship parents.
- Assist service team members and the child in accomplishing service plan goals and ensuring progress towards goals are documented.
- Cover on-call when assigned.
Two years experience with the population served.
LMSW, LPC, LPAT, LMHR preferred.
