Program Coordinator (PC)
Department: Community Residential Services
Reports To: PC Manager
FLSA Status: Non-Exempt
Job Summary:
Accountable for having a working knowledge of programs and opportunities for both individuals supported by Gatesway and the employees supervised in Community Residential Service. Assist the Executive Director of Residential Services in the administrative matters relating individuals and staff under your supervision. Responsible for advocating for individuals supported by
Gatesway to assure their needs are being met and they are receiving supports and services to assist them in achieving their personal outcome as defined within their Individual Plan.
Qualifications:
Must have minimum of Bachelor’s Degree in Social Services or related field, or a minimum of five years in area of Developmental Disabilities services.
Must have a valid Oklahoma Driver’s License with a good driving record.
Must maintain personal vehicle insurance.
Must have a clear criminal record (no felony).
Major Responsibilities:
1. Responsible for the implementation and monitoring the data collection process of the outcomes and methods written in the individual plan and all documentation required completed by staff supervised. Assist employees in the coordination of other service providers and maintaining a working relationship with them.
2. Coordinate with the Executive Director of Residential Services in the interviewing and orientation of all new staff, including familiarization with Gatesway personnel policies and schedules relevant to their shifts. Submit training requests when needed to Training Coordinator to ensure staff is current in all certifications and required training. Develop an appropriate back up plan for the individual’s plan care. Monitor the back up plan, make changes as necessary and notify the case manager of changes.
3. Accountable for Tracking and monitoring budgets and providing reports to the Executive
Director of Residential Services and Case Manager of DDSD. Read and be familiar with the
DDSD policies. Ensure health and safety in the home environments of individuals serviced by
Gatesway. Assist with any conflict resolution between staff, roommates and families. Work with landlord or property owner to ensure compliance with Oklahoma Landlord/Tenant Act.
Visit individual’s home to monitor services and individual’s satisfaction.
4. Report any and all suspected cases of abuse, neglect or mistreatment of any individuals, as per policy. Complete any necessary documentation and present to the Executive Director of
Residential Services. Ensure that legally adequate consent, approval of the Human Rights
Committee, and participation in a positive habitation program occurs prior to implementation of intrusive procedures.
5. Accountable for conducting meetings and in-services with HCC and staff.
Responsible for being accessible at all times to the HCC, HTS and the Executive Director of
Residential Services. Performing on-call functions on a rotating basis for weekend and holidays as assigned by the Executive Director of Residential Services. Perform all other deemed necessary as determined by the Executive Director of Residential Services.
6. In the event the Program Coordinator’s house is short staffed or has an opening, he/she will be required to work any open shifts in order to satisfy the required staffing ratio.

Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Work environment: The noise level in the work environment is usually moderate.
