Qualifications: A minimum of an Associates Degree or 5 years working in the Human Services field.

Day Habilitation Coordinator

This is an exempt, professional management position, subordinate to the Director of Operations; develops directs and evaluates treatment and community support programs, ensures that all aspects of Day Habilitation services are delivered as per the consumer’s Day Habilitation Plan.

Responsibilities

• Coordinates delivery of services with residential program; parents of those living at home.
• Assists in the development of individual team productivity, allocates resource and staff.
• Schedule fire drills, and emergency evacuation drills for each Day Hab participant, chart progress, and maintain records
• Responsible to assist in the consumer and his or her advocate in obtaining all services outlined in the Day Hab Plan.
• Responsible to complete monthly progress notes for all participating consumers
• Maintain all consumer Reshab plans and update each plan as needed.
• Develops goals for the consumer, ensure consumer’s progress, review such progress daily, and trend consumer progress data on a monthly basis, documenting all outcomes, and make appropriate recommendations.
• Attend all Individualized Service Plan meetings, and applicable reviews.
• Ensure the Day Habilitation Program is adequately staffed, based on regulatory ratios.
• Provide direct supervision and guidance to the Field Supervisor.
• Must comply with all other duties as assigned.

Job Type: Full-time

Pay: $60,000.00 per year
