Overview:
KORT Physical Therapy In Home Physical Therapy: Referral Management Coordinator is responsible for maintaining all policies and procedures, day-to-day mechanisms and systems, and routinely communicating with the provider facilities and the client companies to obtain authorization, track and report on the status of In-Home Physical therapy patients. The In Home Physical Therapy: Referral Management Coordinator provides centralized scheduling services to all incoming referrals and serves as a resource for clients, providers, and the In-Home clinical care team. Simultaneously, the In-Home Physical Therapy: Referral Management Coordinator will work closely with the In-Home Physical Therapy: Program Liaisons and Regional Manager to identify and resolve problematic issues and concerns with all parties and systems involved.

The In-Home Physical Therapy Referral Management Coordinator reports to the Senior Rehabilitation Program Liaison of the In-Home Physical Therapy program.
Responsibilities:
  • Obtain authorizations as directed per contract/agreement.
  • Confirm scheduling of each patient as requested.
  • Communicate with providers/facilities routinely regarding patient and authorization status.
  • Maintain and update computer program/daily tracking of each active patient by client, diagnosis, authorized treatment, etc.
  • Provide patient compliance information to case managers and adjusters per agreement.
  • Establish excellent working relationships with client, case managers, and adjusters.
  • Provide ongoing service to In-Home program clients.
  • Provide initial evaluation, progress notes and discharge notes as requested by client.
  • Respond to complaints, concerns, and questions of active clients and providers.
  • Work with all departments and inter-company staff as needed.

Qualifications:
Minimum Qualifications:

  • High School diploma or GED required.
  • Associate or Bachelor’s degree preferred, OR minimum of 2 years of experience in similar role within healthcare.
