We are seeking a Registered Nurse Case Manager to join our team in the Detroit area! You will be responsible for the assessment, diagnosis, and treatment of assigned patients

We currently serve St. Clair, Oakland, Macomb, and Wayne county. We are dedicated to promoting the physical and emotional well-being for each of our clients. Job Duties:

• Provides services requiring substantial and specialized nursing skill, in accordance with the plan of treatment signed by the physician and makes the initial evaluation visit to the client.
• Evaluates and regularly reevaluates, the nursing needs of the client, initiates, develops, implements, and makes necessary revisions to the client's plan of care.
• Observes signs and symptoms and reports them to the physician and/or other appropriate health professionals as often as needed, or upon changes in the client's condition.
• Coordinates the total plan of care and maintains continuity of client care by liaising with other health professionals assigned to the same clients.
• Responsible for following all policies and procedures of the agency regarding service delivery, documentation and care coordination.
• Helps decrease re-hospitalizations by providing education on preventative measures to clients.
• Organize daily schedule by assessing individual client needs and priorities.
• Evaluates effectiveness and outcomes of care.
• Plans for client discharge from services.

Job Requirements:

• Current and unrestricted Registered Nurse license.
• Current and unrestricted Drivers License.
• Active Auto Insurance Policy.
• Minimum of one year general nursing experience preferred
