Envision Home Health & Hospice is looking for a PRN (Per Diem) On-Call Registered Nurse who is passionate about providing quality care to patients and families enrolled in our Home Health program within Utah County, UT.

The RN on-call position provides support for home health patients and their caregivers after hours. On-call shifts begin at 5:00 PM through 8:30 AM Monday through Thursday, and from Friday at 5:00 PM through Monday at 8:30 AM.

Responsibilities:

• Develops and follows plan of care appropriate and specific to patient upon admission.
• Admits new patients as needed
• Provides those services requiring substantial specialized nursing skills.
  
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Informs personnel of changes in the condition and needs of the patient.
  
• Counsels the patient and family/significant others in meeting nursing and related needs.
• Participates in in-service programs.
• Understands and adheres to established Agency policies and procedures.
• Processes orders and notifies physician of patient needs and changes in condition.
• Participates in peer review and performance improvement as assigned.
  
• Gives total patient care as needed.
• Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.

Qualifications:

• Current RN license to practice in the state of Utah
• Current Utah driver's license, reliable transportation and auto insurance
• Maintain current CPR certification
• Minimum of at least one year nursing experience with at least one year in hospice or home health preferred
• Self-directed and able to work with minimal supervision
• Demonstrates excellent observation, problem-solving, verbal and written communication skills, nursing skills per competency checklist
• Shows ability to organize and prioritize workload independently
• Availability during on-call hours and ability to respond in timely manner per company guidelines
