We are looking for Care Managers who will be responsible for the assessment and care management of our long-term care members of any age, enrolled on waiver programs. You must be a Licensed Social Worker (LSW) or Registered Nurse (RN) with a preferred one year paid clinical experience in home and community-based service.

• Proof of influenza and COVID-19 vaccinations are required.
