Job description
Position Summary:
Supports hospice patient care, providing appropriate nursing interventions and collaborating closely with members of the interdisciplinary team. Performs case management and intermittent visits for home and facility-based hospice patients.
Essential Functions:
1. Performs initial and on-going patient assessments for comfort and palliative patient care
2. Establishes and implements an individualized plan of care for each patient
3. Assesses the need for patient and family education and provides education as needed
4. Obtains appropriate physician orders to achieve symptom management
5. Provides safe and appropriate skilled nursing care in accordance with protocols, policies, procedures and physician orders
6. Manages and administers medications according to standards
7. Documents in EMR in an accurate, concise and timely manner per standards
8. Demonstrates efficiency in visit schedule
9. Meets or exceeds departmental productivity standards, including number of visits per week.
10. Determines patient care needs for other team members as appropriate and observes and evaluates the effectiveness of their work
11. Makes referrals to additional IDT members and collaborates with social workers related to the involvement of additional IDT members
12. Fosters IDT collaboration by initiating communication to address PFC needs/updates of plan of care
13. Anticipates potential problems and establishes preventative measures
14. Performs effectively in emergency situations
15. Follows OSHA and agency regulations in the use of standard precautions, the management of controlled substances and biological
16. Provide basic personal care and ADL’s when appropriate
17. Other duties as assigned
Requirements:
- R.N. required; BSN Degree preferred
- NJ RN license required, CHPN or OCN preferred, BLS training
- NJ driver’s license and insurance coverage required
- Minimum of one year nursing experience - medical/surgical, oncology, geriatric or related experience, hospice/home care experience preferred.
- 6 Weekends per year
1 Holiday every other year
