The Grand Pavillion for Rehabilitation and Nursing at Rockville Center is looking for an experienced RN MDS Assessor

The Grand Pavilion for Rehabilitation and Nursing at Rockville Center is a premier facility with 158 beds and is proud of their state-of-the-art Rehabilitation Gym servicing 60 + short term beds. The Grand Pavilion for Rehabilitation and Nursing at Rockville Center team consists of iover 20 years of experience with backgrounds in acute, sub-acute, long term and outpatient settings would provide an excellent setting for learning and growth. The Grand Pavilion for Rehabilitation and Nursing at Rockville Center provides Rehabilitation services 7 days per week.

This Position is responsible for the overall quality, timeliness, appropriateness and completion of resident assessment and care planning in accordance with current Federal, State, and local standards and regulations.

Qualifications:

1. Licensed Registered Nurse in the State of New York

2. Experienced in Long Term Care and Nursing Management

3. Experience with MDS 3.0 scheduling and completion

4. Posses strong communication skills clinical knowledge and skill

5. Experience with CMI case mix and quality measure reporting

6. Demonstrates leadership ability

7. Team player, able to delegate and oversee subordinates and flexible to changing work conditions and situations.

8. Must be computer literate, experience with PCC preferred.

Functions:

1. Oversees MDS completion by all clinical disciplines

2. Completes MDS as warranted

3. Participates in MDS education for all clinical disciplines

4. Schedules and oversees Care planning meetings as needed

5. Meets with families for Care Planning and as warranted by clinical situations or family request.
