Cordova Wellness and Rehabilitation Center, a skilled nursing facility is excited to announce we have a position available for a full time compassionate and qualified RN Infection Preventionist and Wound Care Nurse.

The incumbent provides direct patient care, patient and staff education, qualify improvement activities, research, and administrative duties with a commitment to evidence-based care. The incumbent will serve as consultant for problem solving in nursing and organizational issues concerning prevention of pressure ulcers within the facility.

RN Wound Care Nurse Functions Include the following:

• Leading and organizing deliver of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning.
• Using advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating outcomes.
• Functioning as an expert in clinical practice and/or areas related to the assigned roles and responsibilities.
• Systematically evaluating current practice, and formulating outcomes for groups of patients and/or organizational processes within an area of expertise.
• Provide excellence in prevention, health maintenance, and therapeutic intervention, to persons with select disorders of the gastrointestinal, genitourinary, and integumentary systems.
• Provide quality continuing education in the field of wound care specialty nursing to other health care providers.
• Develops and implements educational programs for patients, staff and other health care providers.
• Performs other related duties as required by Nurse Manager delegated by the Director of Nursing or designee.
• Participates with QAPI committee monthly for performance improvements for clinical outcomes.
