Key Responsibilities:

• Providing direct care in accordance with LiveWell’s 3 C’s of Care philosophy.
• Assisting client with ADLs/IADLs as indicated by care plan
• Meal planning and preparation
• Reporting changes in client’s condition or living environment
• Completes daily log of client visit and submits weekly to RN Supervisor
• Home Management; Home Safety and Sanitization
• Routine housekeeping as indicated by care plan
• Infection Control
• Other Duties as Assigned

Minimum Requirements:

• For CNAs: Current valid Certified Nursing Assistance License
• Must be 18 years or older and able to read and write consistent with job requirements
• Weekend and overnight availability a plus

• Access to reliable transportation
• Ability to pass drug screening & background check.

• Ability to prioritize
• Be flexible, innovative and possess good interpersonal skills
• Ability to work independently, make sound judgments based on care plan and data available and act accordingly

Job Types: Full-time, Part-time

Pay: $12.00 - $14.00 per hour
