Title
Social Work Assistant - BCOM 64 Hours
9:00 AM to 5:30 PM; Every other weekend
Under the supervision of the Social Services Coordinator, the Social Work Assistant performs patient services including, but not limited to, completing the admission with new patients upon their arrival to Bronson Commons, processing and attaching documents to the patient record, and assisting with the patient discharge process and appeal process as needed. Coordinates with other members of the social work staff, and communicates with other staff within the facility in order to ensure excellent care to all patients.
  • High school diploma or general education degree (GED) required
  • Associates degree or some college completion preferred
  • One year of clerical or related experience preferred
  • Ability to successfully complete the organization's competency requirements for English writing, spelling and reading comprehension
  • Intermediate computer skills and the willingness to learn new programs
  • Excellent interpersonal skills
  • Keyboarding and computer skills
  • Excellent customer service skills
  • Light clerical duties as assigned.

Work which rarely produces a high level of mental/visual fatigue, i.e., less than 20 percent of the work time. The job produces some physical demands. Typical of jobs that include regular walking, standing, stooping, bending, sitting, and some lifting of light weight objects. While performing the duties of this job, the employee regularly works in a clean, well lighted & temperature controlled environment. The employee may sometimes be exposed to infectious disease from blood/air borne pathogens. The noise level in the work environment is usually moderate.
The Social Work Assistant must demonstrate excellent human relation skills including verbal and written communication, and empathy skills. Must demonstrate effective interviewing and communication skills with patients and residents
Must communicate daily with social work coordinator and social work team, and with all members of the care delivery team. Demonstrates effective interviewing and communication skills to patient.
  • Complete admission interview and paperwork with patient promptly on admission
  • Handle referrals for patients and maintain documentation of actions taken as requested
  • Assist with patient’s discharge planning as requested by making referrals to community agencies, the patient’s PCP and to other healthcare agencies to ensure that each patient has a safe discharge plan
  • Thoroughly document each case and enters appropriate information into the computer and/or the patient chart.
  • Assist with providing information and referrals to patients and families as needed.
  • Assumes responsibility for professional development.
  • Develop and support a positive work climate and the overall team effort of the department.
  • Assist with determining whether patients and residents have up to date Power of Attorney documentation and provide materials and education to patients and/or their families to complete this documentation.
