The role you’ll contribute:

Performs home visits as ordered by the physician. Prepares social histories. Assesses and evaluates the patient’s socioeconomic and emotional status, family relationships, and coping abilities and their relationships to the patient’s health status and wellness potential. Identifies needs, prepares, implements, and evaluates an individualized Plan of Care. Displays high degree of proficiency in the referral process with appropriate follow up. Provides high quality counseling and resource coordination. Promotes the team approach in the delivery of total patient care. Notifies the physician and other team members of necessary updates. Completes documentation according to Agency Policy and Regulatory guidelines. Provides supervision of BSW visits and plans of care.

• Evaluates patients as assigned.
• Assist the physician and other members of the home health team in understanding the significant social and emotional factors related to the state of health.
• Performs home assessment to evaluate potential for social needs.
• Conducts visits to evaluate patient needs and implement a plan of social service assistance maximizing patient care plan foals and providing for continuing management of needs within the home or seeking alternative arrangements.

The expertise and experiences you’ll need to succeed:

Minimum qualifications:

• MSW Degree from a school of Social Work accredited by the Council on Social Work Education.
• Minimum of (1) year of social work experience in a health care setting
• Current BCLS
• Valid Driver’s License

Preferred qualifications:

• Two (2) years of Home Health Social Work Experience
