The Lead Case Manager is responsible for monitoring a client caseload that is assigned to them. This includes coordinating care for clients, assisting in maintaining medical appointments, connecting clients to resources and treatment planning. This position is also responsible for providing guidance and support to assigned Case Managers, as well as assuming Acting Supervisor duties in the absence of the ACT Team Lead. The Lead Case Manager will also oversee the Representative Payee accounting.
Lead Case Manager Responsibilities include:

  • Ensure service delivery is performed according to the organizations mission, policies and procedures and service philosophy.
  • Ensure compliance with all licensing and state mandated regulations
  • Provide referrals for services to clients as indicated
  • Ensure all necessary reports and documentation are completed in a timely manner (e.g. Treatment Plans, Progress Notes, DLAs, PHQ9P, Pre-Crisis Plans, and OEIs ).
  • Assist ACT Team Lead as needed.
  • Coordinate and maintain designated caseload.
  • Communicate and meet with ACT Team members maintain and assess needs of clients.
  • Act as Interim Supervisor when ACT Team Lead is not in the office. Demonstrate ethical practices in management of staff, resources and services provided.
  • Assist Case Managers with cases as needed; Train staff as needed.
  • Perform other duties as assigned by the Supervisor or Management.
  • Demonstrates cultural competencies in interactions with clients, co-workers and customers.
  • Completes special projects as assigned within designated time frames.
  • Attend various meetings, including treatment team meetings, as well as meetings with referral agencies as needed.
  • Transport clients to various appointments (e.g. medical, dental, probation, attorney, etc.).


  • Bachelor's degree in Human Service field, such as Social Work, Psychology, Rehabilitative Counseling, or closely related discipline is required
  • Computer literacy, including past experience with keyboarding, email, internet, and computerized health record systems is desired
  • Satisfactory driving record and BCI required
