This Summit Case Manager role will work with South Bay patients as well as provide key support to our Venice Family Clinic, a trusted community source of comprehensive, compassionate care. Under the guidance and supervision of the Summit Clinical Manager, you will:

  • Coordinate the delivery of substance abuse treatment services to patients in a primary health care clinic.
  • Discharge Summit patients after substance abuse treatment visits, arrange for follow-up appointments..
  • Provide high quality case management for patients experiencing challenges around homelessness, substance use, and attaining other resources, by linking them and helping them achieve their identified goals.
  • Ensure that patients are linked to care by following up after a missed appointment.
  • Act as a liaison between the medical provider and the patient.
  • Operate in a multi-disciplinary team and have patients referred to them from medical and mental health providers as well as community agencies.
  • Work independently but consult on challenging cases with their supervisor.

Salary range: $20.07-$30.08 Hourly

Qualifications

We are looking for a knowledgeable, caring, and resourceful individual with:

  • Knowledge of HIV/AIDS, community resources, health education methods, advocacy, harm reduction and general clinical knowledge.
  • Ability to create and maintain boundaries with clients
  • Ability and skills with public speaking, leadership, working independently, and work effectively with people of diverse cultural and educational backgrounds.
  • Skill in managing multiple deadlines and demands.
  • Effective communication skills, interpersonal skills, and the ability to discuss sensitive matters in a non-judgmental manner and to respect confidentiality.
  • Working knowledge of diverse cultures and low income populations, including the homeless.
  • Knowledge of HIPPA compliance to client/agency records according to established professional social standards.
