Summary/Objective:

The Swing Bed Coordinator position is responsible for administering the day to day operations of the Swing Bed program. Promotes the physical and mental health of patients by applying the nursing processes. This position is a member of the interdisciplinary team for patient care.

Essential Functions:

1. Coordinates the Swing Bed program and works with the physician and other hospital to determine when a patient is an appropriate swing bed candidate.

2. Provides and reviews swing bed admission documents for accuracy and regulatory compliance.

3. Coordinates, as needed, rehabilitation sessions with multi-disciplinary team members and patients and/or family members.

4. Ensures each patient has a daily swing bed schedule.

5. Develops and maintains a positive and professional relationship with consulting physicians, hospital employees, family members, and visitors as related to swing bed program.

6. Coordinates efforts to meet the personal needs of swing bed patients.

7. Ensures swing bed program meets and continues to meet all regulatory requirements.

8. Coordinates and attends weekly swing bed interdisciplinary meetings regarding treatment of care and progress for each swing bed patient.

9. Assists in the development and coordination of educational needs for inter-disciplinary staff with regards to swing bed program.

10. Provides information for current and any on-going performance improvement plan initiatives.

11. Initiates marketing visits to various case managers to develop and/or maintain on-going referral resources for swing bed program.

12. Assists Utilization Review/Discharge Planner with day-to-day duties as needed.

13. Acts as backup support to utilization review/discharge planning.

Physical Demands:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear.

This position is very active and requires standing, walking, bending, kneeling, stooping, crouching, crawling, and climbing all day. The employee must frequently lift or move objects and patients weighing over 50 pounds.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

Required Education and Experience:

1. Minimum Associates degree in nursing.

2. Current, valid certification in the state of Georgia as a Licensed Practical Nurse.

Preferred Education and Experience:

1. Bachelor’s degree in Nursing.

2. Current, valid certification in the state of Georgia as a Registered Nurse.

3. Current, valid AHA Healthcare Provider training (BCLS and ACLS).

4. Experience with utilization review/discharge planning preferred.
