Position: Transition Coordinator at Achievement Center for Children (Westlake location)
Reports to: Director of Education
Employment Status: 206 Days
Grant Funded Position
Job Goals: The Transition/Vocational Coordinator will:
Qualifications:
1. Three or more years of experience in Secondary Special Education and/or Vocational Education
2. Transition to work endorsement
3. Familiarity of characteristics of students with autism
4. Experience in instructional design and modifications
5. Independent work ethic for self-management of schedule
6. Excellent communication and collaboration skills
7. Professional appearance and demeanor
8. Excellent organizational and writing skills
9. Ability to multi-task, schedule and facilitate both short and long term projects
10. Ability to maintain a positive and flexible attitude toward daily responsibilities, colleagues and schedule
11. Knowledge of the administrative regulations for special education and transition services
Duties and Responsibilities:
1. Coordinate supervision and evaluation of students in vocational training sites.
2. Secure in-school and community job placements for students ages 14-21.
4. Conduct transition evaluations as necessary.
5. Develop appropriate, measurable vocational goals and objectives.
6. Provide written report of student progress at annual review.
7. Complete the Transition Services Component of the IEP and contribute to the Summary of Performance.
8. Conduct individualized transition planning meetings with parents and/or students.
9. Educate parents by distributing information on transition issues, hosting parent information meetings, and scheduling tours of adult facilities.
11. Respond promptly to questions and parent concerns.
12. Maintain required paperwork including but not limited to IEP forms, transportation notices,student data, and classroom correspondence.
13. Participate in the development of the program budget by evaluating the financial needs for transition services recommendations for adequate funding.
14. Provide effective guidance and technical assistance to staff.
15. Work collaboratively with other ACC staff and represent andtatives of outside agencies.
16. Use community resources to enhance instructional programming.
17. Submit reports and other required documentation by the dates assigned.

Terms of Employment: 206 day assignment at a pay rate established by the Board of Control.

- Develop and oversee all transition services for students enrolled in the Achievement Center for Children Autism School. This includes collaboration with the certified and non-certified staff of the Transition Program and interaction with other ACC- Autism School staff.
- Facilitate the achievement of the desired post-school outcomes for students with disabilities. The Transition/Vocational
- Create links for students to vocational and developmental training; financial and residential programs.
- Establish and maintain relationships with local businesses in the community for student access to vocational and daily living opportunities.
