Job description

Position is hybrid. Mostly remote but will require in-home visits. The field visits will be a minimum of 2 days a week onsite and the remaining 3 days will require member visits completed via video call. Manager has some flexibility on which days the field visits can be conducted.
Schedule will be M-F 8:30AM-5PM.
Daily responsibilities will be conducting UAS Assessments to support care management. Candidates will be assigned a minimum of 15 cases per week.
Orientation: Orientation will be 5 days for current UAS certified RNs. For RNs new to UAS, orientation will be approximately 2 weeks and the grad will be assigned a "buddy" to guide them through the process. Management will provide UAS training for recent graduates.

Candidates must be in the New York Metro area (Bronx, Brooklyn, Queens, Manhattan, Westchester County, including Yonkers, Nassau, Suffolk, and New Rochelle). Candidates will have the option to work in 1-2 boroughs.
MUST be bilingual (Spanish, Chinese, or South & East Asian dialects)

Required Experience:
Management will accept EITHER of the following: UAS Certified RN licensed in the state of NY OR recent graduate with an active NY state license. Candidates with a home care background will be better qualified.
Required Education:
Bachelor's Degree or Health Education (a combination of experience and education will be considered in lieu of degree).
Required Licensure/Certification:
Must have valid driver's license with good driving record and be able to drive locally & NY State RN License.
