Job Summary:
The Department of Pharmaceutical Services provide a full range of services for patients cared for by the Medical Center at UCSF as well as its providers and payors. These services include, but are not limited to: drug procurement, storage, and security; safe, accurate and timely preparation and distribution of drugs; clinical pharmacy services for patients; development of drug use policies within the Medical Center; drug information services for clinicians and patients; and participation in technology assessment and quality assurance programs. The Department of Pharmaceutical Services is affiliated with the UCSF Department of Clinical Pharmacy, an academic unit within the School of Pharmacy, which has primary responsibility for teaching professional courses in the Pharm.D. curriculum, research, and public service.

Under the Department of Pharmaceutical Services, Outpatient Specialty Pharmacy provides pharmaceutical care to patients at Moffitt and Long Hospitals, UCSF Ambulatory Care Clinics and Comprehensive Cancer Centers, Mission Bay Hospitals and specialized services to researchers on campus. The Transplant Specialty Staff Pharmacist perform a wide range of pharmaceutical care functions to assure that drugs prescribed for and used by patients are safe and effective. These include, for example, obtaining medication histories, reviewing and assessing medication profiles, providing drug information to patients and health care providers, consulting with prescribers and recommending drugs of choice, ensuring access to medications, counseling patients in the proper use of medications and ensuring that patients understand cautions, adverse reactions, and interactions related to their therapy. The Transplant Specialty Staff Pharmacist will also supervise pharmacy technicians to secure insurance authorizations for medications as well as prepare and dispense prescription medications and pharmaceutical products for patient care. The Pharmacist will be part of the Patient Management Program, clinical activities, and delivery of services as required.
Department Description: The Department of Pharmaceutical Services at UCSF has long been nationally recognized as one of the founding fathers for clinical pharmacy services, which was established in 1965. Today, the department continues to lead the profession through the integration of advanced technology & robotics, comprehensive pharmaceutical care and clinical services.
Required Qualifications:
- Doctor of Pharmacy Degree from an accredited college or university or equivalent experience
- Completion of a PGY1 Pharmacy Practice Residency
- Completion of PGY2 Residency or 3 years of equivalent experience similar clinical practice (>/=70%)
- Computer skills that facilitate Clinical patient data medication order entry and other basic clinical software programs.
- Demonstrated ability to effectively communicate with and educate patients about their medication therapy.
- Written and oral communication skills to clearly and concisely provide information, explanations and instructions to, and to elicit information from others with varying levels of ability to understand, including individuals who are ill or distressed, or who may not speak English.
- Knowledge of current ambulatory care guidelines (i.e. NCEP, JNC, ADA).
- Demonstrated ability to identify medication-related problems in patients, communicate them to a patient's physician and offer recommendations.
- Complete working knowledge of, and skill to apply, practices, principles and techniques of patient care related to area of expertise.
- Decision-making skills to independently exercise delegated authority to act, and to recognize when it is appropriate to seek assistance from others when limits of delegated authority have been reached.
Preferred Qualifications:
- Experience with navigating third party reimbursement programs
- BPS Certification
- Experience precepting, and evaluating residents and students
- Second spoken language preferred
- Management skills
