Position Details
Demonstrate a Rehabilitative/Recovery attitude toward service delivery by providing consumer-driven strength-based approach to case management and crisis services. Act as primary case manager for assigned consumers as demonstrated by management or primary responsibilities including minimal bi-weekly support contacts, completion of consumer-driven service and safety plans, coordination of benefits, housing and all other duties assigned.

Requirements:The ideal candidate will possess the following qualifications:
- Must possess a Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education OR be aRegistered nurse OR possess a high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science AND two years' experience in direct contact with mental health consumers.
- Must possess a valid driver's license and access to reliable transportation
