RN, Shared Hospital/Clinic position, 5 days per week (M-F), Full-Time, shares time between cardiac rehab and clinic for care management responsibilities. Responsible for assisting with cardiac rehab programs such as rehab, Adult Fitness, Stress Tests as well as other cardiac testing, coumadin clinic, and health fair. Also responsible for care managing patients with cardiac disease for the Wray Clinic Care Management Program. Care Management requires care provided through phone calls as well as in-person visits at times. Will report to Director of Cardiac Rehab as well as Lead Care Manager in the clinic. Experience in Cardiac Rehab or care of cardiac patients is required. Requires certifications in BLS and ACLS. Requires good verifiable work history with references, a team player attitude, and be a motivated and creative self-starter to help establish a care management program for cardiac patients.
