Is seeking a

Care Coordinator

Position Summary:

Coordinate multi-systemic community based resources for children and families at high risk for removal from their home or community.

Duties and Responsibilities:

  • Provide intensive community based traditional/nontraditional case management services to identified consumers.
  • Network and collaborate with community resources and providers to insure compliance with consumer’s treatment plan.
  • Maintain consumer records.
  • Coordinate and facilitate specific child and family team meetings.
  • Perform any other department or agency-related duties or special projects as directed by supervisor.

Qualifications:

Bachelor’s degree required, with major in human services preferred.

Ability to work a flexible schedule.

Sensitivity to diverse cultures and language competencies.

Knowledge of behavioral health and case management issues and of providing home bound services.

Good communication and documentation skills.

Ability to prioritize and organize schedules/demands.

Verbal and written communication skills.

Computer literacy.

CPR/First Aid certified or willing to become certified.

Valid driver’s license and access to reliable transportation.
