Case Manager

If you answered yes to any of these questions, the Area Agency on Aging of Pasco-Pinellas (AAAPP) may have the position you’ve been waiting for. The AAAPP is recruiting experienced case managers to help caregivers continue to care for their loved ones, in their homes, and utilizing a network of supportive services.

Job Summary:

Responsible for screening seniors and their caregivers to provide home based support services under the Title IIIE Caregiver Program.

Duties and Responsibilities:

  • Evaluate seniors and caregivers living conditions to identify needs.
  • Provide and coordinate services for seniors and their caregivers that address specific needs while sensitive to budget responsibility.
  • Arrange for service delivery by either contacting contractual vendors or linking the caregiver to existing community resources.
  • Maintain accurate and current case files on each client.
  • Monitor service provision and needs through contact with both caregivers and service providers.
  • Maintain records of clients seeking services, screen and prioritize cases for active status.
  • Keep accurate data in both CIRTS and Excel data bases.
  • Pass Uniform Client Assessment Online Training and utilize DOEA 701 Assessment Instruments.
  • Conduct and/or participate in community outreach, such as health fairs and caregiver conferences.
  • Mentor undergraduate student interns with coordination of client visits and education regarding the caregiver program.
  • Ensure accurate internal and external billing and record keeping.
  • Ensure and ascertain client/caregiver satisfaction with the program through a uniform satisfaction survey process.
  • Maintain client confidentiality, protected personal health information, and ensure secure storage of client records and information.
  • Maintain professional skills for communicating with the public, professionals, and staff.
  • Comply with all security and confidentiality regulations.
  • Stay apprised of recent science regarding Alzheimer’s Disease and Related Disorders (ADRD) with continuing education promoted by the Area Agency on Aging or approved external bodies.
  • Maximize home and community-based services with utilization of additional grant funding to meet needs not funded by Department of Elder Affairs.
  • Any other duties as assigned.

Education:

Bachelor’s degree in social work, gerontology, social services, education, public health, marketing, or a related field from an accredited college or university, or an equivalent combination of education and experience. Case management experience preferred.

Skills/Qualifications:

Good organizational record keeping, communication skills and computer skills, specifically in Microsoft Office Products and database input are required. Must have the ability to establish and maintain effective working relationships with AAA staff, clients, caregivers, and community service agencies.

Special Requirements:

Must pass Level II criminal background screening.

Pay: $22.00 - $24.00 per hour
