JOB SUMMARY

The Workforce Resources Case Manager works with participants with disabilities to assess, provide and coordinate training and services to help a participant identify suitable jobs and career paths to help them obtain and maintain competitive integrated employment.

ESSENTIAL DUTIES/RESPONSIBILITIES

  • Conduct vocational intakes and assessments to help a participant develop a Participant Service Plan considering the participant’s expressed interests, strengths, level of functioning and accommodation needs based on knowledge of the disability.
  • Select, administer, and interpret assessment tools and work samples, which address academic achievement levels, aptitudes, interests, physical/emotional abilities, and work readiness.
  • Interpret evaluation results and write reports providing logical recommendations regarding the vocational rehabilitation needs of the participant and participant’s potential to successfully function in a job placement and/or training program.
  • Facilitate staffings with participants, primary supports, and referral sources to discuss participant’s progress in-services.
  • Consult with employers and other stakeholders to facilitate access to employment. Compile and maintain list of community outreach with employers. Identify and coordinate vocational training opportunities to help a participant prepare for employment.
  • Provide job placement support to help participants with job search, submitting applications, following up on job leads, interviewing and on-boarding. Develop job leads using a variety of sources;
  • Contact potential employers in person, by telephone and through mail to generate interest in BDI employment services.
  • Provide information to employers on hiring disabled persons such as ADA incentive programs, management &supervisory techniques like job coaching. Accompany participants to interviews to observe and help clarify questions if needed.
  • Provide on-site job coaching to assist a participant learn job tasks and to address any concerns with the participant or employer. On-site job coaching may require evening and weekend work hours.
  • Completes follow-up evaluations with employer and client monthly (through 90 days) after placement to ensure successful job performance. Records data in Therap regarding each participant within 24 hours of service provided.
  • Monitor, document, and report participant progress in accordance with BDI policy, referral sources and CARF.

REQUIRED EDUCATION AND EXPERIENCE

  • Master’s degree in Rehabilitation Counseling, or other directly related discipline
  • Two-years of related experience providing direct job placement and supports to persons with disabilities.
  • Current registration as a Certified Rehabilitation Counselor (CRC), or eligibility to sit for the certification examination.

ADDITIONAL ELIGIBILITY QUALIFICATIONS

Must pass background screens.

ACRE (Association of Community Rehabilitation Educators)

Certification for Customized Supported Employment.

Knowledge of applicable state and federal laws and regulations

.Knowledge of GVRA, DBHDD, CARF, and any other regulatory and licensure standards.

PHYSICAL DEMANDS

Must possess the physical capabilities to perform the following: standing, walking, bending and sitting for extended periods.

POSITION TYPE/EXPECTED HOURS OF WORK

This is a full-time position with expectations of a minimum 40-hour work week. Occasional evening and weekend work may be required as job duties demand.

TRAVEL

Must be able to travel as needed to provide services.

Pay: $45,000.00 - $55,000.00 per year
