Case Manager that has a passion for providing the highest quality of care with compassion and integrity.
PRIMARY FUNCTION:
The Case Manager is a member of the treatment team, functioning as the primary coordinator with clients in order to establish all aspects of discharge planning.
SCOPE:
Under the immediate supervision of the Clinical Manager, the Case Manager is responsible for each component of the client’s treatment planning, client’s behavioral health plan, including all programmatic implementation. Case managers will also coordinate aftercare plans such as MAT service appointments, mental health continuums, primary physician appointments, relapse prevention support within the community including NA/AA meetings and identify transportation for client at time of discharge. The Case Managers will also meet with their assigned case load individually every 7 days to adhere to documentation requirements through Medicaid standards.
GENERAL RESPONSIBILITIES:
The Case Manager must adhere to HIPAA compliance in all communication regarding clients. Competency in usage of DSM-V. Case Managers will coordinate Discharge Planning, Peer Recovery, Housing Assistance, Employment Assistance, Benefit Assistance (i.e. food stamps, insurance), and Coordinate Transportation along with other items as needed. Documentation is essential as responsibilities include weekly Progress Notes, developing Relapse Prevention Plans and identifying Individual Aftercare Planning. Competency in usage of DSM-V and documentation and reporting in EMR.
MINIMUM QUALIFICATIONS:
- Bachelor’s Degree Preferred in a Behavioral Health discipline or related field
- The ability to prioritize and multi-task, with exceptional time management skills.
- Ability to competently encourage and inspire direct reports in the delivery of exceptional clinical services through professionalism.
- Demonstration of a client-centered team approach, with advanced computer skills.
- Scheduling flexibility REQUIRED to accommodate varied client admissions.
- Understanding of Substance Use Disorders and Dual Diagnosis
- Reliable transportation is required
SCHEDULING REQUIREMENTS:
Full-Time, M-F 9a-5p or 10a-6p
COMPENSATION:
Hourly- $16.50-$19.00
