CPST [Community Psychiatric Supportive Treatment] Case Manager -
Warren, OH,

Salary $: Full-time; 40 hours per week position includes a competitive salary and benefits package.

Shift: Dayshift, M-F

Responsibilities:

  • Needs assessment and formulation of the ISP
  • Individual therapeutic interactions/rapport-building
  • Crisis stabilization
  • ADL prompting, coaching and teaching
  • Client feedback
  • Decisions pertaining to clients’ need for additional services, resources or more restrictive settings
  • Choice of treatment interventions, techniques, advocacy activity and frequency of contacts
  • Pursuit of collateral contacts to gather additional psychosocial information when indicated
  • Adherence to agency’s risk assessment/management protocols
  • Documentation is clinically accurate, substantive, timely and complete and confirms to program
  • Interactions with essential others are constructive and serves to advance the progress in treatment, the quality of care and the agency’s standing in the community
  • Participates actively and productively in agency meetings and supervision sessions
  • Knowledge of community resources

Qualifications:

  • Bachelor’s degree preferred. Previous mental health or community support/case management experience required. Experience with severe and persistent mental health diagnoses, substance use disorders, and dual diagnosis
  • Knowledge of community resources is critical.
  • Ability to use a desktop computer, electronic health record for documentation, a landline telephone and access to a cell phone (agency or personal) for accessibility by staff and clients. Experience with telehealth video platforms (Google Meet) and EHR (Credible)
  • CDCA is required; candidate will need to have prior to hire or will need to obtain within 45 days of hire

Miscellaneous requirements: Criminal background check; Pre-employment drug screen; Clear driving record; TB test and First Aid/CPR. Enthusiasm, initiative, self-motivated, organizational skills, ease in establishing rapport, ability to effectively communicate with the population, cultural sensitivity, willingness to provide services in the community including client homes.
