Case Managers in the Service Coordination and Navigation (SCaN) Program serve Chicago youth ages 14-24 who have a history of interaction with the justice system or have been impacted by violence, helping to stabilize them in their environments, increase self-efficacy, and assist in their transition to adulthood. The Case Manager completes a thorough assessment and provides a range of services including educational reengagement, employment assistance, career exploration, and connections to additional community resources and supports, all with the goal of reducing community violence and improving youth outcomes.
Staff are be required to be fully vaccinated against COVID-19 and to submit proof of their vaccination status before their start date unless they have an agency approved medical or religious exemption. What you’ll do:
- Complete intake and individualized assessment for assigned youth.
- Provide comprehensive case management services aimed at helping assigned youth reach their individual goals.
- Monitor process toward clients' goals and objectives and make recommendations for additional service linkages when necessary.
- Work collaboratively with other systems, including Juvenile Justice and educational systems, to meet the needs of clients.
- Complete accurate and timely documentation of services
Who you are:
You have at least a Bachelor's Degree in Social Work or a closely related field or a high school diploma/equivalent with three years’ related experience.
- Experience with clients who are at high-risk of community violence or involved in the Juvenile Justice system preferred.
- Understanding of the impact of trauma on youth and families highly desirable.
- You are able to meet with families in-person, in their homes or other community locations, and you have a valid driver's license, insurance, and a reliable vehicle.
- You are an intuitive, nonjudgmental listener who easily builds relationships built on respect, trust and positive regard.
The details:
- Location: Community-based services are provided in the Greater Englewood, Chicago Lawn, and New City communities. Home office located at 5958 S. Marshfield in Chicago.
- Schedule: Full-time, hourly; Monday-Friday, general business hours with flexibility to serve some families during evening/weekend hours as needed.
- Salary: Range starts at $17.93/hr; offers are commensurate with experience
