Full-time or Part-time Clinical Nurse Practitioner is needed in the Opioid Treatment Program, operated by Suffolk County Department of Health Services, Division of Community Mental Hygiene. Nurse Practitioner at the Opioid Treatment Program would provide Medication Management to patients, including assessment of patients seeking treatment of Methadone and assess the appropriateness for this level of treatment.

Job Duties:

· Initiate Methadone induction and provide ongoing Medication Management for Methadone.

· Conduct Health Assessments to make a determination on status of patients, as well as medical service needs. Assessments would include diagnoses, assist in the development of Service Plans and ensure patients would receive the required care within accepted standards of practice.

· Gather and interpret patient history, assess for diagnoses, provide care for acute and chronic mental illnesses, provide education, counseling and guidance.

· Follow up on patient's bloodwork and refer accordingly when warranted to patient's medical diagnoses and needs.

· Provide Crisis Intervention to patients when required.

· Makes referrals to other health care providers and/or consults a physician as needed;

· Does related work as required.

Essential knowledge, skills and abilities:

Thorough knowledge of the principles, techniques and practices of the medical field to which the incumbent is assigned; thorough knowledge in assessment, making diagnoses and prescribing medicine and treatment within the clinical specialty; thorough knowledge of current literature and other sources of information and recent developments in the field of specialty; ability to tactfully obtain accurate information through interviews and observation, and to work with the general public and other governmental agencies; ability to understand and carry out complex oral and written instructions; ability to instruct patients and staff members; ability to perform physical examinations and evaluate findings accurately; ability to effectively collaborate with a multi-disciplinary staff regarding patient treatment/care; ability to establish and maintain effective work relationships with others, ability to maintain accurate records and charts; physical condition commensurate with the demands of the position.

Minimum Qualifications:

  • Valid certification as a Nurse Practitioner (NP) issued by, and registered with, the State of New York, with a specialization in a definite area of practice recognized by the State of New York.
  • Infection Control certificate (required by NYS)
  • Valid DEA
  • Addiction Services: 1 year (Preferred)

Work Schedule: Monday - Friday (6AM - 2PM), Tuesday-Saturday (6AM - 2PM) or Flexible between 6AM -2PM.
