POSITION TITLE: Coding Specialist I

DEPARTMENT: Health Information Services

CLASSIFICATION: Non-Exempt

JOB SUMMARY:

The Coding Specialist I should have working knowledge of the ICD-10, HCPCS and CPT coding systems. The coding specialist is responsible for translating diagnostic and procedural phrases utilized by healthcare providers into alpha-numeric codes. Current job duties include being able to complete the following with limited immediate supervision: Review, analyze and interpret the entire electronic medical record for the current date of service to identify all diagnoses and procedures documented during the visit.

ESSENTIAL JOB FUNCTIONS:

The following is a summary of the major essential functions of this job.

The incumbent may perform other duties, both major and minor, that are not mentioned below; and specific functions may change from time to time.

1. Determine and assign the principal and significant secondary ICD-10-

CM diagnosis codes and CPT-4 procedure codes, using official coding

guidelines and knowledge of anatomy and physiology, pharmacology,

and pathophysiology/disease processes.

2. Identify cases with clinical indicators that may require provider

documentation clarification and/or specificity in order to

accurately assign codes

3. Abstracts pertinent information from patient records. Assigns ICD-

10-CM or HCPCS codes, creating APC assignments.

4. Suggest and assist with workflow process improvements as

appropriate. Participate in coding quality and productivity

processes.

5. Acting as the HIM Technician who will be responsible for the

collection of charts from the clinical staff, scanning the

documents into the proper Electronic Medical Chart, indexing these

files and analyzing for deficiencies.

6. Responsible for tracking the physician deficiencies and notifying

the physician, HIM director and administration if there is to be a

suspension due to incomplete charts.

7. Accountable to maintain an open communication line with the

clinical staff and physicians directly to the HIM department.

8. Assists patients with setting up and maintaining access to their

Health Information Portal accounts along with assisting with

Release of Information as needed.

9. Clerical duties including in this position are not limited to being

responsible for checking the mail and allocating it to the

addressed HIM team member daily, answering phones with proper

etiquette, using standard office machines such as copier, scanner,

fax, and phones.

6. Performs other job-related duties as assigned by supervisor.

7. Complies with Corporate Policies, Departmental Policies and

Procedures, and Personnel Employee Handbook.

8. Demonstrates excellence in customer service skills.

9. Fosters an atmosphere of teamwork and collaboration.

10. Participates in quality improvement activities including serving on

quality improvement teams.

QUALIFICATIONS:

The following qualifications, or equivalents, are the minimum requirements necessary to perform the essential functions of this position.

EDUCATION AND FORMAL TRAINING:

Educational requirements for this role are RHIA, RHIT, or CCS in addition to previous experience in a similar role of a health care setting. The use of Meditech Expanse and Magic are preferred but not required.

WORK EXPERIENCE:

One to three years in a medical records department.

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:

This position should be proficient in the use of Excel, Word, and Adobe. Position must be a highly organized, self-motivated, and dependable component to add to this department. This role must understand the intense use of HIPPA and always maintain patient privacy.

PHYSICAL REQUIREMENTS:

Must be able to being able to sit or stand for long periods of time and lifting 25 pounds or more at once.

EXPOSURE CATEGORY:

Category III. Tasks that involve no exposure to blood, body fluids or

tissues.
