Established in 1980, the Greater Lawrence Family Health Center (GLFHC) is a multi-site mission-driven non-profit organization employing over 600 staff whose primary focus is providing the highest quality patient care to residents throughout the Merrimack Valley. Nationally recognized as a leader in community medicine (family practice, pediatrics, internal medicine, and geriatrics), GLFHC has clinical sites throughout the service area and is the sponsoring organization for the Lawrence Family Medicine Residency program.
GLFHC is currently seeking a Community Health Worker / Medical Assistant (CHW/MA) working for the Moms Do Care (MDC) program. The Moms Do Care (MDC) Community Health Worker / Medical Assistant (CHW/MA) is part of an integrated team that addresses the clinical and social needs of obstetric (OB), post-partum and parenting patients with Substance Use Disorder (SUD). The CHW/MA is responsible for applying their knowledge of community resources and culture to assist the OB and SUD team in fully assessing and identifying a patient’s needs through a comprehensive care plan. This position supports the MDC Registered Nurse (RN) by performing nursing support and clerical tasks, case management, outreach, education, social support and linkages to appropriate supportive services through coordination and navigation.
- Must be able to don appropriate personal protective equipment (PPE).
- Performs accurate assessment of vital signs and other pertinent data and documents in the Electronic Medical Record (EMR) during clinical sessions.
- Participates in intakes and assessments and assists MDC RN in development of a comprehensive care plan with a focus on the social determents of health.
- Links patients through supported referrals for Behavioral Health services and community resources and provides short-term follow up visits or phone calls.
- Coordinates appointments with providers and ensures timely delivery of treatment and navigation services.
- Tracks medical and behavioral appointments and develops and implements appointment reminder strategies.
- Remains current on all referral sources, including but not limited to: housing, nutrition services, peer programs, Mass Health/Healthy Start, advocacy services, transportation, parenting skills and groups, prenatal classes, substance abuse and mental health services including methadone maintenance, domestic violence, employment and training, nursing case management, and primary care.
Qualifications
- Graduate of a Medical Assistant program (or evidence of education and experience working in a foreign country as an LPN or RN).
- Degree in Human Services preferred or combination of education and experience in community based service delivery considered.
- Bilingual, bicultural (Spanish/English) required.
- Community Health Worker Certificate or must complete with 12 -18 months of hire.
- Become a Certified Application Counselor within 2 months of hire.
- Computer proficiency, especially in Microsoft Word, Excel and Outlook.
- Valid Massachusetts Driver’s license and access to reliable transportation.
- Current CPR/BLS Certification.
- Ability to work evening hours.
