Join a team to provide services to adults with severe and persistent mental illness living in the community. The (Community Rehabilitation Services) Community Integration Worker/Case Manager is responsible for delivery of direct mental health case management services as well providing other client’s psychosocial rehabilitation services while they are living in a group care environment. This will be done through participation in an interdisciplinary team model to develop Individual Support Plans with consumers and provide a variety of services and support to assist them in meeting their identified goals. The Community Integration Worker/Case Manager develops 1:1 therapeutic relationships that encourage and support consumers in their own recovery. The Community Integration Worker/Case Manager provides these services in the client's home (Shalom Housing) and in the community. The Community Integration Worker/Case Manager identifies community resources with clients, makes appointments and referrals as well as facilitates and coordinates with other providers to ensure continuity of care. The Community Integration Worker/Case Manager also assists residents in carrying out skills of daily living and in utilization of community mental health supports as well as having knowledge of and assisting with the action steps of our residents current Individual Treatment Plans. Other duties include medication administration and/or supervision, overseeing and maintaining physical plant including housekeeping, maintenance, safety and supplies. This role also provides direct services/support to clients living in the Group Home part of the program. CRMA training will be provided.

Associates degree in social services or related field with 1 year of successful community-based mental health work preferred. Degree requirement may be substituted for additional experience if have MHRTC. Must be MHRT-C certified or provisional eligible.

Applicants must have a valid Maine driver’s license with acceptable driving history. We do criminal background checks and driving history checks on all potential new hires.
