Avatar Home Health was founded in Spring, TX in 2006, Our primary mission is to provide our patients with the best quality Home Health & Hospice care. We are committed to treat our patients and their families with the utmost respect, dignity, and care.

We are currently looking for a home health and hospice community liaison in the Houston Medical Center. The position would be responsible for outreach, assessing potential patients, transition support, and care coordination. Responsibilities include development and promotion of home health/hospice programs and services while maintaining the standards of practice consistent with quality health care and maximizing human, financial, and equipment resources.

The position includes salaried pay, mileage, paid time off, and potential for bonuses.

Job Qualifications:

Education - Graduate of an accredited college/university is preferred.

Experience - Experience in the home health/ hospice industry is required.

Skills - Ability to establish and maintain effective working relationships with all segments of the staff. Ability to travel is required you will be the community liaison for the Houston Medical Center and our office is located in Spring, Texas.

Job Type: Full-time

Pay: From $40,000.00 per year
