Job Description:
The Community Support Specialists provides a vital resource to individuals with complex medical and behavioral healthcare needs as part of the Case Management team. The Community Support Specialist is a point of contact for member’s engaging in case management services offered by Security Health Plan (SHP). Through outreach and collaboration, the individual determines the member’s needs and how to best address them, in collaboration with the Care Management team (RN and SW). The Community Support Specialist works as part of the Care Management interdisciplinary team assisting members in successfully managing within the community. The Community Support Specialist educates members on benefits of the program, assists with assessment of the member for their healthcare needs and assists the team with day to day management of member needs identified on an ongoing basis, ongoing outreach to members telephonically, primary contact for member questions and scheduling of team assessments and member follow up.
This position is a remote position. All training is currently being done online.
JOB QUALIFICATIONS
EDUCATION
For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.
Minimum Required: High school diploma or equivalent. Successful completion of Mental Health First Aide training within one year of hire.
Preferred/Optional: Associates degree in sociology or human services education. CNA certification Successful completion of Mental Health First Aide training at time of hire.
EXPERIENCE
Minimum Required: Experience working as a psychiatric technician, certified nursing assistant or personal healthcare worker. Strong non-verbal and verbal communication, strong coping strategies, engagement of basic problem solving strategies, how to establish and sustain self-help and educational groups. Demonstrated keyboard, computer, and telephone proficiency.
Preferred/Optional: Experience working with individuals with complex medical and mental health conditions.
CERTIFICATIONS/LICENSES
The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.
Minimum Required: State of Wisconsin driver’s license and acceptable driving record.
Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire.
Preferred/Optional: Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) at time of hire.
Remote - work from home position. Preferred Wood or Portage County, but not mandatory.
