Hopewell Health Centers is a nationally accredited federally qualified health and community mental health center with 23 sites across TEN counties throughout Southeastern Ohio (Athens, Franklin, Gallia, Hocking, Jackson, Meigs, Perry, Ross, Vinton, and Washington). We are currently seeking an Adult CPST Provider to work with our Geriatric/Medically Fragile clientele at our Athens, OH location.

As an Adult CPST Provider servicing the Geriatric and Mentally Fragile, you will be responsible for delivering quality, person and family-centered, medically necessary, rehabilitative, culturally-competent Community Psychiatric Supportive Treatment interventions including but not limited to: assessment of needs, skills building/empowerment, crisis management and intervention, advocacy, education/training, eliminating barriers, coordinating/linkages, and monitoring. In collaboration with person served and/or family/legal guardian, develops an individualized service plan (ISP) for each person in the program to achieve behavior change, increased quality of life, and the best outcomes possible for the people we serve. Generates a current, legible integrated clinical record (ICR), in which documentation reflects a clear clinical picture for assigned person served. Maintains minimum acceptable billable expectation as designated by agency UR sheet. Follows billing procedures as required by agency policy. Completes correctly and submits in a timely manner per agency policy documentation, billing, time sheets, mileage, and, as required by supervisor(s) work and appointment schedules. Attends as scheduled and participates in supervision, team meetings, professional development, First Aid/CPR training and serves on assigned agency committees, as appropriate. Informs supervisor(s), client rights officer, or human resource director, as appropriate, of any complaints from persons served, families, members of the community, or workforce members; suspected workforce member abuse/harassment towards persons served or their family/significant others or other workforce members. Abides by agency code of conduct, policy and procedures and mission statement (e.g., business practices, clinical, personnel, and plan manuals) at all times on agency property or while conducting agency business. Performs other related duties as assigned by supervisor(s), program director, clinic director, and agency administration.

What do we look for in our Adult CPST Providers??

  • HS diploma or Equivalent (BA is Social Work or Psychology preferred)
  • LSW, LPC, RN preferred
  • A minimum of 3 years verifiable Behavior Health experience
  • Demonstrates the skills and ability to communicate and work with persons receiving service, their families, community service providers, and team members in the assessment, problem solving, and delivery of rehabilitative services
  • The ability to effectively communicate verbally and in written form
  • Ability to work as part of a multi-disciplinary team
  • Good assessment and intervention skills
  • Previous experience with EMR/EHR strongly preferred
  • Demonstrates understanding about the needs of clients who have severe and persistent mental illness that impacts their ability to manage multiple and chronic health issues effectively.
  • A valid driver’s license, automobile insurance, good driving record, and suitable vehicle for transportation of persons served
  • The ability to assist persons into and out of a vehicle.
  • Previous experience working with Geriatrics/Medically Fragile clients
