Full Job Description

General:

In accordance with outlined regulations, monitors and supervises the outlined daily routines of the assigned residence. This includes but is not limited to established environmental, medical, and fiscal needs of individuals. Contributes to the development and implementation of treatment and required documentation within the specified time frames. Responds to crisis situations expediently and provides the appropriate action and support necessary to resolve the issues.

Specific:

• Establish a secure, positive relationship that nurtures client to feel comfortable to express needs, self-expression, and goals.

• Monitor and maintain a clean, healthy, and safe environmental condition of the residential setting.

• Aid client in the home and in the community such as work, learning and leisure.
• Provide individualized care for clients in accordance with their Person-Centered Plan.
• Encourage client’s independent decision-making, productivity, and participation in a range of activities.
• Maintain knowledge of each resident's limitations and requirements.
• Communicate resident’s progress with service providers as outlined.
• Complete and maintain documentation in compliance with company procedures.
• Transport client to and from appointments, volunteer opportunities and leisure activities
