Do you communicate well with multiple people in a variety of settings? Are you passionate about patient care? In this role you will represent Halcyon Home in the community, completing starts of care and care oversight of patients for two of our preferred Assisted Living Communities. Minimal travel!!

Your role will be the Halcyon Home clinical representative in the community to coordinate services, provide care, and prepare clinical documentation. Ultimately focusing on providing effective communication to patients, family members, team, and other healthcare professionals.

Home Health experience is essential, along with a proven ability to manage a fast paced, independent work schedule. Knowledge of senior living communities and WellSky/Kinnser software would be great!

Job Type: Full-time

Pay: $78,000.00 - $83,000.00 per year
