Health Disparity Regional Social Worker/Case Manager

Location(s): Livingston, Tangipahoa, St. Tammany, St. Helena and Washington

Position Summary:

This position will report to the Statewide Health Disparity Social Worker with the responsibility for ensuring that residents within the region are connected with resources to foster health equity and eliminate health disparities related to COVID-19. Incumbent will identify regional resource gaps and develop thoughtful approaches to bridging the gap to improve health equity and health outcomes. Incumbent must be a registered Social Worker and will be a member of the Regional Preparedness and Response Team lead and directed by the Regional Medical Director with day-to-day supervision provided by the COVID-19 Coordinator with direct reporting to the Health Disparity Statewide Social Worker.

Core Functions:

  • Provide resources and education within the community to improve and increase testing among populations at higher risk and that are underserved, including racial and ethnic minority groups and people living in rural communities.
  • Provide consultation and coordination of regional programs related to health equity, health disparities and COVID-19. Collaborate with Regional Leadership and Regional Response team in regards to the delivery of social services related to COVID-19. Work collaboratively with COVID-19 internal partners (i.e. Regional Health Disparity Strategist/Planner, etc.) to ensure alignment of strategy and mission.
  • Represent the agency in partnership with local social service organizations. Lead a collaborative with regional organizations to advocate and facilitate for services for children, families affected by COVID-19, and underserved populations. Participate on the OPH COVID -19 Health Care Disparity Advisory Council to inform current and future strategies for reducing health care disparities.
  • Provide linkage to resources and care coordination services to promote health equity and the elimination of health disparities.
  • Provide regional case management consultation as it relates to COVID-19.
  • Provide regional training and workshops that address health disparities and improve health equity.
  • Performs other related duties as required.

Minimum Qualifications:

Bachelor’s degree and 5 years experience working as a registered Social Worker and 4 years experience using intermediate functionality of Microsoft Office.

Preferred Qualifications:

Master’s degree in Social Work. Training/certification in health care diversity, equity and inclusion.

Job Type: Full-time

Pay: $22.00 - $28.00 per hour
