Job description

• Support the flow of home health and hospice admissions, promoting efficiency, outstanding customer services, and attention to detail
• Data entry of referrals into the electronic medical record system
• Verify insurance, obtain authorization and schedule visits
• Carefully review medical records to assure appropriate referral information is obtained
• Perform telephonic outreach to physician offices to obtain complete referral information
• Interface with patients to support the scheduling of home health and hospice assessments
• Coordinate patient care (Orders, schedule, supplies, meds etc)
• Communicate with all disciplines, including Registered Nurses, Licensed Practical Nurses, Physical Therapists, Occupational Therapists, Speech Therapists, regarding the scheduling, frequency, duration, amount, and scope of services, as well as patient care updates.

Pay: $16.00 - $23.00 per hour
