Hospice Regional Clinical Manager
Job description
Manages the Patient Care Coordinators assigned to specific regions. Oversee the overall on-boarding process for patients admitted onto hospice program within the regions. Runs IDT meetings to determine plan of patient care and patient disposition.
Requirements
1. Current NYS RN License
2. Current Driver's License.
3. A Baccalaureate Degree in Nursing from an accredited school is required.
4. A minimum of 7-10 years’ experience in nursing which includes minimum of 5 years of hospice.
5. Management experience required
6. Working knowledge of hospice regulations and Conditions of Participation
7. Must be able to travel to assigned regions ( Bronx, Manhattan, Nassau/ Queens/Brooklyn and Westchester).
8. Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions.

Salary Range: 145K - 150K
