Job description
  • Receive and track all treatment referrals from APD and APD funded community partners who make referrals to Harbor Lights for drug treatment services and the Recovery Wellness Center.
  • Complete assessments and intakes for clients who are referred to Harbor Light by APD or APD-funded community partners. Assessments may be in jail, at the Community Assessment and Services Center, or onsite at the Harbor Light Center.
  • As requested by APD, conduct outreach and host information sessions about treatment services at the Harbor Light Center to Deputy Probation Officers and other justice and community partners.
  • Triage candidates into Harbor Light (interview, screen, psychosocial assessments, and determine candidates’ capacity to live in a social-model transitional living center).
  • Complete candidates’ intake process within the established timeline, as indicated by APD.
  • Coordinate the participants’ orientation to the program(s), most often within 72-hours.
  • Oversee a smooth transition between detoxication and treatment, including any referrals that might be necessary.
  • Attend and participate in scheduled case conference and case review meetings, staff meetings, and training; represent The Salvation Army Harbor Light Center in the community.
  • Communicate and collaborate with Counselors, Program Director, Executive Directors, Residence Team, community partners, and service providers to provide optimal support to program participants.
  • Maintain all obligations under HIPAA, Title 42 of the Federal Code of Regulations, and Title 22 of CA Code of Regulations.
  • Advanced reporting using a computer and various data-tracking software.
  • Assist the Program Director and Business Director in the preparation of reporting requirements for APD.
  • Act as a representative of the Executive Director in internal and external meetings as required.
  • Assist the Executive Director in current and planned strategic partnerships with future expansion opportunities for the Harbor Light Program, including Private Insurance Companies, Medi-Cal, Outpatient, and more.
  • Work with the Divisional Social Services Director and the Executive Director in maintaining high standards with the Agency’s National Social Services Standards.

Qualifications, Knowledge, Skills, and Abilities:

  • Required:
    • Two years’ experience in substance abuse and/or mental health services.
    • Detailed and results oriented.
    • Valid CA Driver License.
    • Must not be on active parole or probation.
    • Written and Verbal command of the English language.
    • First Aid and CPR certification (within first fourteen days of employment – company provided).
    • Tuberculosis clearance (within first seven days of employment).
  • Proficiency with Microsoft Office and various other office processes applications desired.
  • Ability to be professional, punctual, flexible, and reliable.
    • Ability to work within a frequently changing project scope while maintaining overall direction and structured priorities.
  • Integrity and ability to handle sensitive information in a confidential manner.
  • Excellent organizational skills and ability to multi-task with follow-through.
  • Desired:
    • Bachelor’s Degree in Behavioral Sciences (Social Work, Humanities, Psychology, etc.).
    • Culturally competent and able to work with a highly diverse population.
    • Experience working successfully with issues of substance abuse, mental illness, and other potential barriers to self-sufficiency.
    • Bilingual preferred.
    • Knowledge of clinical documentation.
    • Knowledge of Medi-Cal Delivery System.
    • Knowledge of co-occurring disorders, gender-responsive, and trauma informed treatment preferred.

Physical Requirements:

  • Ability to grasp, push, pull objects such as files, file cabinet drawers, and reach overhead
  • Ability to lift to 25 lbs.
  • Ability to operate a telephone.
  • Ability to sit, walk, stand, bend, squat, climb, kneel, and twist on an intermittent or sometimes continuous basis
