ORGANIZATION: Citizen Care Inc.
DEPARTMENT: Community Living (Residential Group Homes)
POSITION: Community Living Nurse (LPN)
STATUS: Full-Time; 40 hours / week
HOURS / SCHEDULE: Monday through Friday, daylight / early evening hours; every sixth weekend
LOCATION: Greentree/South Hills
WAGES: $25/hour; increases with years of service
Citizen Care Inc., a subsidiary of Partners For Quality Inc., provides residential, day program, habilitation, supported employment opportunities and behavioral support services to adults with intellectual and developmental disabilities. We are a non-profit organization, accredited by the Council on Quality & Leadership. Citizen Care has locations throughout the West Hills and South Hills of Allegheny County.
The Community Living Nurse is responsible for providing medical care, supports and services to people with intellectual / developmental disabilities. Duties include, but are not limited to:
- Communicating health care concerns/needs to the appropriate people
- Focus on The Councils Outcome Based Performance Measures
- Compliance with the 6400 Regulations
- Having a working knowledge and understanding of outcomes, goals and Plan for Services and Supports
- Assuring that a safe and healthy environment is maintained, including adherence to peoples rights
- Assisting people in maintaining optimal physical health, through observation and prompt, appropriate action
- Coordinating the routine delivery of health care services together with site management/staff
- Assuring health care summaries, MARs, med/nursing notes and the social medical histories are accurate
- Assisting people in scheduling appointments, communicating with their physician, reporting their symptoms and receiving appropriate treatment for their health-related concerns
- Accompanying people on medical appointments as needed/requested
- Providing people with skilled nursing as needed/requested
- Reviewing all peoples medical status and submitting written reports to the Team as required
- Membership in nursing related committees as assigned
- Training of site staff related to peoples medical diagnosis/symptoms as needed/assigned
- Reviewing and responding to Incident Reports of a medical nature
- Maintaining all medical, nursing and other records as required
- Exercising initiative and educated decision making
QUALIFICATIONS
- Must be a Licensed Practical Nurse (LPN) with a current Pennsylvania license
- Two (2) years experience in nursing required
- Experience in Rehabilitation Nursing desired
- Experience in dealing with emergency and/or acute medical problems
- Excellent references, both personal and work
- Act 34 and (if applicable) FBI Clearance
- A valid, PA, Ohio or West Virginia drivers license
WAGES
$25/hour; increases with years of service
