Job Responsibilities:

  • Medications: aiding in training of new Caregivers, and reviewing medication books
  • Working with resident doctors according to needs, and ordering prescriptions from pharmacy
  • Writing and speaking on ISP's (Individual Service Plans) for residents within 30 days of move in, and keeping them up to date with 6 month checks, or according to resident condition changes
  • Thorough review of all homes on the following: medication book, med cart, and resident needs being met
  • Monitor Caregiving staff charting to ensure quality and proper communication about resident statuses
  • If needed, attend doctor and/or psych appointments with residents
  • Join our Management On Call Schedule according to scheduling needs, and be a reference for staff questions regarding residents on those days

Additional Requirements:
  • Have a valid Driver's License and reliable transportation to and from work sites throughout Southeastern Wisconsin.
  • Have a valid, active LPN license.
  • Have a minimum of 1 year of experience in an LPN role, with experience in home care preferred but not required.

Location: office headquarters in the city of Port Washington
Hours: full-time
Salary: $25 per hour
