Summary of Position:The Care associate will help aide in all back office work for field based staff Alegis Care providers. This includes general patient care coordination duties, phone call handling and routing, and orders management duties. The office based care associate will support all lines of Alegis care business including chronic care management, transitions of care, and Palliative care programs.
Duties & Responsibilities:
Support and manager workflow for assigned providers
Communicate with all providers of any Abnormal /Critical results
Escalate patient concerns to the appropriate medical teams
Coordinate internal task to insure that tasks are completed daily
Assist Patients with any refill requests and medical needs
Arrange consults and care services for assigned patients
Ability to handle a high volume of incoming calls and the routing and messaging of these calls
Participation in patient care rounds and interdisciplinary care team meetings as indicated
Assist with the tracking of patients hospitalizations and discharge home when applicable
Follow up on all patient care orders including labs, diagnostics, skilled nursing, consults, and durable medical equipment. This will include outreach and establishing relationships with medical vendors
Knowledge, Skills & Abilities:
Familiarity with electronic medical record documentation
Strong communication skills
Excellent customer service skills
General medical office knowledge, exposure to medical terminology and skills of a medical office administrative or medical assistant position
Comfortable navigating insurance
Proficient in Microsoft word, excel, and office

Certifications:
High school graduate or equivalent
Medical Assistant Certification required
Work Environment
Must have the ability to work in a fast pace environment
Must be able to work in a team setting
Office based business hours- Monday –Friday 8am -5pm
If you will be working at home occasionally or permanently, the internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload.
For this position, we anticipate offering an hourly rate of 17 - 25 USD / hourly, depending on relevant factors, including experience and geographic location.
This role is also anticipated to be eligible to participate in an annual bonus plan.
