Position Purpose: Responsible for timely activation of Health Home clinical staff to provide services to minors removed from their home by the Arizona Department of Child Safety (DCS), follow-up, documentation and reporting services. Responsible for timely activation of Health Home clinical staff to behavioral health inpatient facilities for coordination of care, engagement, enrollment and referral for services. All services are tracked, documented and reported

Position Responsibilities:
Monitors email boxes and accepts telephone calls for activation of Health Home services to children removed from their homes by Department of Child Safety (DCS) and services to the foster care placement and/or for activation of a provider to respond to a behavioral health inpatient facility for coordination of care, enrollment, engagement, and referral to services

Dispatches clinical team for DCS minors whose jurisdictions are in the Cenpatico Integrated Care geographic area and placed in another region

Verifies membership enrollment and current services being rendered in multiple data bases/systems

Documents all dispositions and follows-up on dispositions not received in the designated time frame. Enters data in multiple systems and databases

On a rotating basis, provides evening, weekend, and holiday coverage by logging into system, activating referrals, documenting, and tracking activations and dispositions

Maintains acceptable clinical documentation, provider contract information, DCS referrals, and other information

Reviews monthly report tracking activations and dispatches for compliance and works to resolve identified related issues. Works to resolve timeliness of provider dispatches and follows-up on related issues

Education/Experience: High School Diploma or GED and 1-2 years of experience required. Bachelor’s Degree and experience in the healthcare industry preferred
