Job description
$25 per hour
Mentor - Part Time (2 hours per day) either 3:30pm to 5:30pm or 4pm to 6pm depending on the school dismissal for after school programing.
1. Facilitates program activities for youth ages 11-17 who participate in BYC services;
2. Assist Program Coordinator by formulating and coordinating individual needs assessments, goal setting, face-to-face and phone conversations on a weekly/bi-weekly basis, family engagement, linkage with community agencies, and follow-up services for youth and participants as needed;
3. Work to assist youth to determine what factors put them at risk of engaging in or becoming victims of aggressive behavior and other anxieties;
4. Help to determine what methods each youth should use in order to avoid risk-taking factors and reach their goals;
5. Reach out and re-engage youth that has inconsistent attendance, work with families to bring resources to the home, and connect with programs;
6. Conduct data collection and coordinate reporting to funders, marketing staff, and other stakeholders;
7. Utilize the IDHS-required eCornerstone internal data system to ensure program data are properly entered and exported for the program and administrative needs.
Requirements:DUTIES AND RESPONSIBILITIES:
1. Implement other services requested by SGA or the Illinois Department of Human Services;
2. Enter case notes, assessments, and case plans in a timely manner as well as respond to e-mail communication, and all other required information;
3. Assist with data entry tasks, including transferring paper-based files to electronic databases;
4. Update and organize files as needed;
5. Assist with basic maintenance of agency data system and demonstrate strategies learned for best usage of eCornerstone (if time allows);
6. Displays sensitivity and responsiveness to cultural differences present in the service population;
7. Reach out and re-engage youth that has inconsistent attendance, work with families to bring resources to the home, and connect with programs;
8. Review monthly data reports and assess appropriate benchmarks;
9. Research and develop recommendations for data visualization tools and solutions to aid program and administrative staff in using and interpreting programmatic data;
10. Coordinate “data dialogues” with program staff as a part of quality improvement efforts.
11. Manage and maintains the required caseload of clients throughout the duration of their participation in the program;
12. Work closely with and provides referrals to other community-based services for youth experience violence at school or in the home;
QUALIFICATIONS AND REQUIREMENTS:
1. An associate’s degree from an accredited school in social work, psychology, public health, or related field, and two years of experience working with at-risk youth;
2. Displays sensitivity and responsiveness to cultural differences in the service population.
