Job Summary:

The Patient Advocate acts as a liaison with patients, families, and staff. Provides service recovery and resolution to patient and family complaints and grievances. Provides support and helps to problem-solve issues and concerns for families and patients. Acts as a coach to leaders and team members throughout the organization on Person and Family Centered Practices and Service Recovery at the point of contact.

Responsibilities:

• Communicate the hospital mission, services, policies, and procedures to patients, family members, visitors, and staff
• Assist patients, family members, and visitors in navigating through the hospital systems to achieve positive outcomes
• Make daily rounds to greet new admissions and/or follow up with patients, families, and visitors previously encountered
• Primary responsibility for patient/family complaint process including intake, investigation, and resolution
• Facilitate appropriate and timely communication with patients and families acting as liaison between patients, family members, and health care team members to address potential or actual patient care issues.

Requirements:

• High School diploma required, Bachelor's Degree preferred
• Experience and familiarity with Electronic Medical Records
• Experience coaching preferred
• Excellent communication (written and verbal) skills needed
• Good problem-solving/critical thinking skills
